A letter would be best...other wise just tell the boss you are leaving in two weeks

Personally I would write it using a computer and print it out and then hand it to him or her. I would say something like. Thank you for the employement you have given me at what ever place you work at and then say something like. I have learned so much from working at this company and with all the employees and thank you for being being so kind and patient with me. With all this said I regret to inform you that I have found another place of employement and I am now in writing giving my two weeks notice.
I would do something along those lines. Only say good things and try to be positive when talking with your employer, even if you did have bitter feelings with the manager or the employees. Do not feel like you have to explain every detail. If someone did something to offend you silence is the best answer, they will figure it out. And if you are leaving for better pay and to do something you love then that is something positive and you do not have to concern yourself with what I just said. I would just say to your boss when you hand them the paper. This is my two weeks notice and let them read and then they will probably ask a couple hundred questions and then you can elaborate about why you are leaving. Hope this helps :)

Write a letter stating that you are leaving the organization to pursue other opportunities. You can find samples letters by googling for one or searching for one on monster. I would send the letter to your immediate supervisor and possibly to HR. If you are leaving on good terms or during a very busy season, you may want to come up with a transition plan. A transition plan document everything that you are working on, the processes that you use, related files and contact, and who will be do that working once you leave.

One Tip- Exit interview are optional and you never have to disclose where you are going.

talk to your boss, and have it in writing. a simple, "sir, i'm puttnig in my two weeks' notice" should suffice, but if the boss presses for details, be polite. don't trash the company if you want a good reference. do the same thing in your letter--say you're going in a different direction, or whatever your reason is, as long as it isn't mean or offensive.

Write a letter,stating that you are giving your 2 week notice.You don't have to put why.Make sure to date it.Keep a copy for yourself.Hand it in to the "head"person.Be sure to sign it also.

I have always spoken, in person, with my employer, regarding my two-week notice. Or, you can write a letter. Do whichever feels more comfortable for you.

go to your supervisor and tell him or her you are giving your 2 weeks notice, you can even write it up one for your sup and one for yourself dated so if anything happened you have proof you told them
