That's a trick question. "Neo" itself was his hacker name inside the Matrix, which, ultimately he discovers to be his true identity. There is no last name to a hacker name (i.e. "Trinity" or "Mouse"). His name in the Matrix was Thomas Anderson, but Neo winds up being his true name, but this is actually an anagram meaning "one", as in "the one" or the fulfiller of the prophecy.

Neo had no last name. He had only the name of who he was while in the matrix itself. This is used throughout the film in order to dissuade Neo from his belief in Morpheus' theory and in is his own growing strength in being "the one". All characters have only the name that they have taken as their own or had bestowed upon them: Neo, Morpheus, Trinity, Mouse, Tank, etc. The names that they have, or had, inside the matrix program never truly existed and thus were not their names from the beginning.
