What kind of relationship do you guys have with your ex's? If the relationship is good, and your fiancé and her ex are still friends, so what if she sent him some x-rated pic... I send x-rated pics to my friends sometimes. But are they "friends" for the right reasons, or is it more like someone doesn't want to "let go" after all this time - it's up to you and your fiancé to work out. Perhaps, your gut feeling is right, and she's still hanging on. Talk to her.

I would be....if she is calling you by his name,then she still maybe has feelings for him!? I would talk to her about it and see if,maybe she still has feelings for him? now,If she calls out his name instead of yours during sex..... then he is still on her mind besides you.
I would just talk to her about it though and tell her it is bothering you.
