I'm looking for an actual 49ers license plate, not one of those fake ones but an actual one. Not looking for license plate frames but an actual license plate.I have seen 1 car that had this plate and I can't find it anywhere.

Have you tried Ebay? I typed in "49ers license plate" and got a number of hits, many of which are actual metal plates, not just the frames. Here's an example of one, but there were many more:

How do you tell a "real" one from a fake one? If you expect to find one online, how will you know?
Assuming that as a 49er faithful you are from California, please remember that you still have to have your regular California license plates on the FRONT and the back of the car. If anyone tries to tell you that is not true, he is dead wrong.
Cops are not going around, wasting a lot of time pulling cars over for not displaying the front plate. But, if an officer stops you for, say, forgetting to signal, or for a busted tail light, he is going to check out the rest of the car and you WILL get, at least, a fix it ticket for the missing plate.
