As everyone has said, he shoots himself in the cheek so it just leaves a hole there. He does not die in the book. In the last chapter, he does talk about God and such but God is actually a doctor and he is in a hospital. He mentions graveyard shifts and people bringing him food. And "God" has diplomas on his wall. People write to him, including Marla.

If you read the book, he actually does die, and the last chapter is about him in heaven. It was changed in the movie version, so that he wouldn't die. He actually shot himself in a place where he had been picking it continously and a hole formed, but that wasn't explained at all in the movie.
