you pay 10 percent of the adult fare for a child under 12 months and do not get a seat for the child.(has to be carried on your lap for the entire flight).

Some airlines will let them have a seat if you pay between 50 and 80 percent of the adult fare. (varies among different airlines, but Alitalia, the Italian airline does not offer this option).

They only go free when travel is entirely within North America

Not at all, just enjoy the travel till your child is 2 year old. Some airlines allow till 3 years of age. Check with the airlines.

Depends upon who you are travelling with most of the no frills airlines eg easyjet, bmibaby etc dont charge you however most of the charter companies eg first choice thomas cook etc charge you a token fee (anout £30 rtn). You are best of checking with the airline however the one thing you do need to remember is that whether or not you pay for them all airlines expect you to advise them (so dont forget like one of our customers did and then kicked up a stink when she was told they werent on the manifest and so couldnt go - we sorted it fortunatly for her) however unless you pay full fare they dont get a luggage allowance except for a buggy or travel seat (which is put in hold) they wont get fed but most airlines will heat up baby food for you. Once they are over two most airlines charge full fare.

Only if you want a separate seat for them. Otherwise it's free, but you have to have the child on your lap.

On a long haul flight, what would having a child on your lap do to the chances of you getting deep vein thrombosis? Hmmm...

No children under two travel for free. You will need a separate passport for the child, you can not put them on yours the way you used to.

depends on which airline. we just got back from aruba, and on all international flights, even with the child sitting on a parents lap, the child must have a paid ticket.

Flights are usually free, they must sit on your knee with a special lap belt around them, that link's to your belt.
They do not get a meal.
