For Mary Kay consultants or above?

I am a Senior Consultant. I want to put up a fish bowl with a sign "Enter to win $20 of FREE MARY KAY! I need some sort of entry form (my business cards are on order, so I don't have the customer cards yet). What should I include on the form?

The slip should have a place for their name, address, phone number, best time to call and maybe a yes/no question if they currently use a skin care system.

Try what they do in restaurants sometimes, where you have them drop their business cards in. Have a small flier that they can take with all of your contact info (name, phone, website, and email)
