None. Humans rarely take personal responsiblity for any serious problem. There has yet to be a war where the loser (or winner) said "Sorry, we were wrong. The war was our fault". At best, we look at history and might say our ancestors were wrong, but usually even then we provide excuses.
Other common examples: Alcoholism - disease, not a fault; Gambling, disease, not a fault; Gas shortage - gouging, not overdependence; Crime - poverty, not personal choice; Unwanted pregnancy - Guy, not the girl (or visa-versa); Evil - Devil, not personal choice; Fired - boss, not performance; Fat - McDonalds, not personal eating habit; Unruly kids - Schools, not family values; etc.
It's not human to truly admit or even recognize when we are our own problem.

America invades a soverign nation like Iraq to get rid of the bad guys. We reduce the country to pre-civilized levels: no water, no power, hospitals destroyed, no leadership or police. One day we wake up and realise what the people really need to be happy is for us to leave. And we're the ones who toy with middleeastern politics and helped put Saddam in power. The problem is us.

I really believe that we, as a people, are tremendously self-destructive. There are so many scenarios to support your statement that it is extremely difficult to pick just one.
It is estimated that smoking cigarretts will kill up to one billion people, worldwide, by 2100 A.D., but do they stop smoking?
NO!
Obesity will kill 30% of the population 10 to 15 years earlier than normal in their lives. Will they stop overeating? NO!
Alcohol will kill millions. Will they stop drinking? NO!
Sincerely, I can go on from this point to infinity without reaching any end to the self-destruction we impart upon ourselves.
