She needs to go to a doctor, preferably a gynecologist. At 49 this may just be an indication that she is BEGINNING menopause. Can she get pregnant now? yes it is possible. If she has taken two pregnancy tests and both have come back negative she most probably is not. Menopause starts many changes in a woman's hormonal balance so the test strips may be wrong and she may indeed be pregnant. It happens. Does she want to be pregnant?
IUD -- Yes, unfortunately this happens more times than you would think. This is very unfortunate because the embryo may implant close by the IUD and as the embryo and then the fetus grows it may grow around the IUD. This can cause many things, early spontaneous abortion (miscarriage) which it is designed to do, thereby preventing a viable embryo. But, if the embryo does not abort other things can cause problems. Uterine infection, and of course if the fetus continues to grow it can be disfigured and have other very serious defects and complications. Again this lady needs to see her gynecologist very soon.
Her doctor can tell her whether she is in menopause, if her IUD may be out of place, or if indeed she is pregnant.
Please, please encourage her, go with her if possible. This if very important to her own health.
I know this sounded cold and clinical and I'm sure she has strong emotions about this situation. I encourage you to be there for her. Listen to what she tells you. HEAR what she is saying. To some people a baby is a baby from conception. (I'm one of them) and this can make for some very hard decisions. Support her whichever choice she makes NO ONE knows what they would do in the same situation unless they have been there. Even then, everyone has different circumstances in their lives they must base their decisions on. I will pray for her and you. Please give her my best. Pregnancy rates with IUDs are low.
Age is not an indication of menopause. If she's still having her period, she's not in menopause and could still become pregnant. It's also possible she's in perimenopause, which can make her periods irregular.
She should see a doctor about her unexplained pain.

Can you get pregnant with an IUD? Yes. Can you get pregnant after menopause? No, not if she really is in menopause. Once you go into menopause, you are no longer ovulating which is required for pregnancy.
I suggest she see her gynecologist to see what is causing her symptoms.

I am a pediatric nurse no you can not get pregnant on the IUD sounds like she may have a urinary tract infection and the tenderness and heaviness in her breast might be from a hormone change of going thru menopause and if she experiences anymore problems she needs to see her gyno
