Your percentage load to number of reps should be to failure. 90% of max weight load will be about 5 reps 95% 3 reps and 100% 1 rep 3 days a week recovery time 2-3 minutes. Try doing a circuit where no 2 consecutive exercises exercise the same muscle group.

Low reps, high weights - like do no more than 4-6 reps to failure x3 sets. Supplement with creatine, helps swell your muscles up. And eat plenty of low-fat protein.

Or there's always steroids - but those have some awful side effects. There's no really quick way to get more muscle mass, you just have to keep at it.
