I would like my site visitors to leave comments on pages of my sites. How do I do this?

I want visitors of my website to be able to leave comments on the pages. I also want to be able to delete inappropriate and spam comments.
Anyone know how to do this?
I have Dreamweaver and my site is able to handle PHP, MYSQL, CGI, and many other formats.

I am not a form+MySQL database put together person. Detailed information should be provided..... :)

And why do people keep thinking I need a guest book?
I am not looking for a guest book, I am looking for a comment system. I want comments to be shows at the bottom of my HTML pages. People can leave comments, and then they are seen on the page. Not people leaving comments in a guestbook. I already have a guestbook.
