In the united states you call surgeons and physicians Doctor.
They are surgeons after passing the United states medical licensing exams 1, 2 (composed of a clinical skills and clinical knowledge section) and 3. Subsequently you take the relevant surgical Board exams and become FACS (fellow of american college of surgeons) this can further be followed by a fellowship and a fellowship certification...
In the UK after gaining your MRCS (membership of the royal college of surgeons) be it the England, or London or Edinburgh section ensued by the FRCS (Fellowship of the Royal college of surgeons) again they are divided into london, england or edinburgh colleges. Physicians gain the MRCP and FRCP.
After gaining your FRCS you are a qualifies surgeon traditionally about 8-12 years after graduating from medical school (MBBS, MD or MBChB which takes anywhere between 4 and 6.5 years depending on where you study)- you become a Mr. Ms, Mrs, or Miss again (i.e you lose the doctor title) this is due to traditional reasons, since back in the day surgeons were "barbers" and apothecaries (who made drugs/medicine) where doctors/physicians.

they are called Miss, Mrs or Ms depending on what they are normally called.
Its all to do with the history of the training of doctors, since surgeons used to be barbers, and not go to medical school at all, so werent called 'doctor'. Nowadays, all types of doctors go to medical school, but surgeons use the title Mr, or Miss, in honour of the barber-surgeons of old.

Male surgeons are "sir" and female surgeons are "ma'am". If you're on a first name basis, then you call them all "Doctor".
