It kind of depends on the pitchers "rank" within the rotation. In most cases, a five man rotation has five pitchers that pitch on every fifth day, but when a team has an off day in the middle of a five game period, the #5 starter might get skipped, so that the #1 or #2 can pitch five days after his previous start. Not all teams treat this the same way, but it's logical to use your best pitcher (a #1 or a #2) over your fifth starter if you can do so...

Sometimes either way. Let's say a team has a five-man rotation. Pitcher #4 pitches on Sunday. The team has Monday off. They can either use Pitcher #5 on Tuesday (which would mean he went 4 games between starts) or Pitcher #1 (which would mean he went 4 days between starts). Most teams will use their #1 pitcher as much as they can, so they skip the #5 guy. But if a team is worried about its pitchers throwing too many innings, they might use Pitcher #5 and give everyone else an extra day of rest.
