It can mean several things. Firstly, I have to STRONGLY say you need to consult a doctor. here are some things you have to think about.
cough with blood = Hemoptysis.
vomit with blood = Hemetemsis.
blood in feces = Hematochezia (fresh blood), melena (dark altered blood).
shortness of breat = dyspnoea.
difficulty swallowing = dysphagia.
painful swallowing = odynophagia.
1. Any history of travel to areas with endemic tuberculosis?
2. Is it fresh blood or altered/rusty coloured specks? What's the quantity?
3. Fresh blood in small quantity could mean that you might have coughed really hard, having a nosebleed, scratched back of throat etc. or an upper esophageal bleed.
4. Coffee ground blood or rusted specks etc. suggest a lower esophageal or gastric ulcer/bleeding varices. the acid in the stomach has altered the blood which is now coming up, or you might have swallowed blood from bleeding nose which got altered and now coming up.
5. Is it always blood associated with cough or is there bleeding with no cough?
6. Any associated shortness of breath or chest pain? Any history of smoking?
7. Tests which your doctor (and i have to stress YOUR doctor) which might have to be considered are a chest x-ray, a sputum detailed report and culture (specially an acid fast bacilli smear for tubercullosis), sputum for cytology (to see any abnormal cells or cancerous cells), a "tuberculin skin test" if you are not from a TB-endemic area.
If you have not already gone and seeked medical advice then do so TODAY. I hope it is not a serious medical problem, but if it turns out to be I wish you a good recovery.
Sincerely,
-Dr. M.

As the previous person said...it means you need to go to a doctor, but I know you wanted to hear more than that ...these are the things that come first to my mind as possibilites...
Severe pneumonia..., an espohageal injury...did you recently each something that scratched or hurt badly as you were swalllowing it ? , bleeding of any causation in the stomach...usually ulcers or injury....and lastly
cancer.
Consider ...was this a one time incident...is it a lot of blood or just blood tinged ? ...
See a doctor asap

that somewhere in your respiratory tract you are bleeding, it could be from the back of your mouth to the base of your lungs, color, consistency and amounts indicate better where it might be coming from.....suggest if you are coughing blood, you see a doctor. It could be something easily dealt with, or something truly serious...the doctor will know.

You could have an infection. It sounds like something very serious, and you need to go see a doctor.
