You look at it under a microscope. Take it to any jeweler at all, and they'll probably tell you for free if it is real or not.

real and cultured pearls will have a rought texture exterior, plastic knock off will be slippery.
Real pearls have layers inside (think of an onion) and cultured pearls do not. The only way to know for 100% certain is through an xray or if the pearl already has a hole for strining, a fine fiberoptic light and a magnifying glass may be enough to reveal the layers.
Even experts can mistake a real (acutally known as "natural") pearl vs. a cultured pearl. Especially in the market today.

When I was little, my mom used to say that if you rub a pearl against your teeth if it is gritty like sand it is real, if it is smooth it is fake.
