Doctors are on very tight time schedules, and have more important things to do than watch people get ready for a clinical exam. While you are undressing, he/she is reviewing a chart, ordeing a test for someone just seen, answering a phone call, confering with office staff, etc. Also, some people are uncomfortable undressing in front of others, and physicians generally respect that. Modesty is innate as well as learned/reinforced in the american culture. Except in very extreme cases (e.g., coding in an ER, certain surgical procedures) few, if any, patients are asked to be completely naked in front of any health care provider -- that's what those paper gowns (or cloth in a hospital) are for -- patient modesty is expected.

Its simply a matter of courtesy. Watching a female undress, or being watched if you are a female undressing, can be a bit awkward for either party because of the intimacy involved. Granting privacy to the patient while she undresses hopefully minimizes the discomfort factor of the exam.

It's respect. Yeah it's funny because he ends up seeing you naked anyway, but that doesn't mean he has to sit there and watch you get undressed.
