Its actually the difference in the octane values of the petrol. Extra premium has got a higher octane value than normal petrol which increases the fuel efficiency.

The only difference that I know of is the octane rating of the fuel. The higher the octane rating the better it is for high compression engines. Sometimes the lower octane fuels can cause your engine to ping or run poorly. For the majority of the users out there regular petrol is good for all. All of the petrol companies have their own special additives that they get the suppliers of the fuel to put in the gas for their pumps. Here in Western Canada we only have 4 fuel refineries that supply gas or petrol as you call it. Petrol is made from crude oil. The fuel efficiency is how your vehicle burns the petrol in its normal operation and what ratio it gets its power from the petrol. Most fuel stations get their fuel hauled in by tanker trucks and it is dispenced to the consumer by electric motor pumps or in some cases by hand pumps at gas stations around the world. You have some good answers here, higher octane fuel is required by some engines usually due to high compression engines, the higher the octane the slower the fuel burns thus reducing ping (predetonation). Predetonation is the fuel igniting in the engines cylinders prior to the spark plug igniting the fuel, this causes the fuel igniting in 2 places in the cylinder and causing the ping noise, ping is bad for the engine, newer engines have anti-knock (anti-ping) sensors that will adjust the engines timing to reduce or eliminate ping if it is occuring but if your engine is still pinging you should go to a higher octane fuel, you don't have any benifits of burning a higher octane, so burn regular unless you have some knock-ping or if your engine requires premium

Extra Premium merely means that the gasoline is a higher octane than regular gasoline. It is refined identically, just for a longer period that boosts the octane, or ability of the fuel to ignite correctly at a high compression. Normally, high compression engines, mostly found in high performance and luxury cars, require a higher octane gas than say an economy car. Using lower octane gas than what is recommended can result in loss of performance, knocking, and sometimes engine damage, however using higher than needed octane will not boost the performance of an engine, it merely wastes your money. Therfore, only use the octane recommended by the manufacturer.

The difference is the "octane level" of the gasoline, or the extent to which it is refined. Octanes are hydrocarbon compounds. Premium gasolines are "purer" more concentrated forms of refined petrochemicals. They burn cleaner, and more efficiently and contain less non value added compounds. They cost more at the pump because it is more difficult and expensive to produce them. Whether they are more fuel efficient I am not totally sure, but it stands to reason that they are by some margin. Fuel stations are supplied by trucks that deposit the fuel in underground tanks.

In extra premium they give more average and more mileage, more pick-up.
