The smallpox vaccine makes a scar if your body isn't already immune.
People who worked with cattle, who had been exposed to cowpox, were made immune to smallpox, so they didn't get the scar.
I had to get this vaccine in the early 70's to go to Europe. I must have really needed it, because my arm hurt like hell for days, and I got a little round scar.

It was the small pox vaccine.
The reason it left that mark is that it wasn't given as a "shot" but with a two pronged needle dipped into the vaccine solution. The needle is then used to prick the skin a number of times, usually in the upper arm.

Small pox vaccine - it causes a scab on your arm and then may leave a scar- the scar can resemble a bite mark
TB skin test is not a vaccine, but if performed on someone that has TB it can cause a large induration or bump on the forearm where the test is performed. In severe cases it can leave behind a permanent scar.

Only two vaccines leave a permanent scar at the site where they are injected (usually the upper left arm). These are to protect agains Smallpox and Tuberculosis. The scars are different, but fairly similar in appearance to non-medical people.
Smallpox vaccine is now only given to those in the military (who are being deployed where biological weapons may be used) and to certain health-care workers who might encounter smallpox in the course of their work.
BCG vaccine (against TB) is not regularly used in the USA but is used in most other countries as it is very effective in preventing TB.

I believe it was small pox. It was given back in the 30's. Some other vaccines also leave scars.

depends where on the arm the scar is. If it is between the wrist and elbow, than its most likely TB. If it is three little marks than it was small pox, and it its one big needle scar it was probably tetnis.
