There are several reasons. First, there are a number of websites like 6pm.com (formerly shoedini.com). Then there are also so many mega stores popping up all over the country. It is more cost effective for the shoe companies to allow other stores to sell their stuff and not pay for the overhead associated with owning a building (rent etc..).

www.zappos.com , and the like. Also, have you been in a shoe store recently in the mall? They usually have a couple size 5's in stock and maybe a 14 or two. Plus slightly lower than expected personal quality.
