The correct answer is: #1 The end of the toilet paper coming over the top of the roll.
It looks nicer and you have better control of how much you pull off.

Etiquette says behind the roll. I say whichever seems good for the bathroom. The one at my house needs to go over the front. At my parents house it's better that the paper is in the back.

Over the top. That's the way they designed it. If you look at patterned toilet paper, the pattern will show as it's unrolled. If you have it from behind the roll, as it unrolls, the backside is showing.

As long as it goes down the drain without backing up, it doesnt matter how the roll is put on the dispenser.

with the paper (prefferably quilted) falling forward off the roll towards you, it is easier then to take a sheet with a sharp tug without having to hold the roll with your other hand. thus preventing cross contamination
