I will answer this as I am a transsexual. I have not had sexual reassignment surgery yet, but I have had my NAME legally changed, not my gender marker.

Honestly, it varies depending on state as far as changing the name on your birth certificate. Some states will allow a complete change, and other will only allow it to be amended.
As far as the driver's license goes, if you are LEGALLY considered to be of the opposite sex from which you were born, and by "legally" I mean you have went in front of a judge with a letter from a physician and/or surgeon stating that you have undergone sexual reassignment surgery (and this does NOT necessarily have to be surgery of the genitals. For female to male transsexuals, it can simply mean a hysterectomy and/or a double mascetomy. All I need is to have my breasts removed and I could be "legally" a male) You have to take that letter in front of a judge who then can make the determination as to whether you are legally male or female now. If the order states that you are now the opposite sex, you have to take that information, the order, the letter from the doctor and other identifying information to the driver's license facility. Some give you weird looks, others think nothing of it, because it is a LEGAL document. They cant' fight it. They might not LIKE it, but it's a court order. So then, yeah, you get a new picture with a new name and gender marker on it. In my case, I just went to get my name changed on my driver's license and I guess they just assumed because of the way I look, that I was a male, and they accidentally put "M" on my driver's license instead of "F". Not that I"m complaining! I LIKE it that way. It saves me a lot of hassle, but it's not legal.

The gender on the driver's license will represent the legal sex of the driver. Laws vary by state as to what documentation is needed to change your sex designation on your DL. In almost all cases of complete transition and post-op surgery, the "new" sex will eventually be indicated on the driver's license of a transgendered person.

If they go through all of the processes and forms to get their gender legally changed, then their new driver's liscence will show their new sex.

I think it may depend on whether or not the state that they live in allows the change of gender on a birth certificate.

its a long process of changing it and you have to go to court and have had your doctor to legally diagnose you as the opposite sex, then he gives you a letter and you can go and have your DL changed to male or female, but as for a name change that's diffrent and cost about 250$ for your entire name to be changed

I got a new license when I changed my name when I was transitioning and a new birth certificate, not corrected, but new after my SRS.
