I am under the assumption that he is scratching something private. I think the best way to be polite about it is to privately joke around with him regarding his habit. I think he would respond better to that then if you were to be rude to him, since he might not even realize how uncomfortable it makes you feel or that he's doing it noticably. I think being honest and saying that it makes you feel uncomfortable is also good, but at the same time, I think he would be more embarassed to hear, "I'm sorry, but I don't like it when you scratch your ". It's like telling someone that they smell.
Anyway, I think if you joke around about it, he'll either get embarassed and stop because you noticed, or he'll keep doing it, in which you can then pull out all the mean jokes about his crabs in front of your co-workers and boss. Hahah. See if THAT stops the itch!

Tell him maybe he is allergic to something for scratching all the time, depending on where the scratching is taking place. If its in his private place, he may have crabs, you need to tell him to go to the doctor. Tell him you have noticed he may have a problem to seek medical advice.
