Like every other profession - the cream rises to the top. Its also a question of having luck, lots of talent and a few breaks along the way. No different than becoming a movie star - have a bit of talent and alot of determination and there you go.
The top writers make so much because they have a style of writing and unique ideas (something that the public wants to read) and they are therefore in demand. They can then demand more money. Also, deals are made that move them up in earnings too---most of them have literary agents and that helps too.

well, sort of. It's a trade-off...because most journalists can get steady work, often with benefits--even if it's just at a local paper. So the pay may be low, but it's safe But there are probably millions of starving aspiring writers--and the likelihood of smash success is about the same as trying to break into Hollywood. Most writers spend their lives as waiters, etc., just to pay the bills. It's a very risky career, and those tend to pay well when the risk is worth it.

Because most people aren't great writers or spend years trying to find a job. If they write a book they have to devote years to that and then more time and money to getting it published before they actual start getting the money.

Supply and demand. Writers who generate a lot of interest (and most importantly, sales) will make more money.

I would say its a lot like acting, it pays great if you can actually make a name for yourself, but compared to the number who try, there are only a small number who do.
