you may need a new one, or it could be the phone. If you have another phone from the same provider put your sim card in it and see what happens. if it works then the phone is broken. if it does the same thing in another phone the sim card is defective. If you haven't damaged the sim card in any way and it just malfunctioned, your provider should replace it for free. if you damaged it or fried it by entering a password incorrectly too many times, you will need to purchase one. if you have t-mobile they are 19.99 plus tax and yes you can keep your same number

If the SIM malfunctioned on it's own and not because of you, it won't cost you a thing. Go back to your service provider and have them replace it. I had that happen with my T Mobile phone last year and they replaced it free of charge. In the event you have to buy one, they normally go for $25-$50 depending on your carrier.
