No, this has never been done. Theoretically it should be possible in the very very remote future, but it would be extremely complicated. You would have to produce all of the molecules necessary to sustain every single biochemical process. Even in extremely simple cells, this is currently well beyond the means of synthetic biochemisty and even knowledge on how to do so.
There are people working on a minimalist genome of the cell with what is thought to be the smallest genome, Mycoplasma genitalium (I think). This organism has about 500 genes which scientists are systematically knocking out to find the smallest amount the cell requires (appears to be around 350). Nearly all of these are thought to be required in every single organism from this simple creature to man.
If you knew this simple genome, and you could synthetically create this DNA in the lab (we can make quite large amounts of DNA synthetically in the lab, but I'm not sure this big), synthetically produce the proteins required to get the gene regulatory program off the ground, and place these in a bilipid membrane (which we can also make) in a suitable growth medium, then the cell should survive (possibly even divide). This would be the first ever example of man creating life.
It will happen someday, whether it is in the USA (amid growing fear of science and dogmatic religious zealots) or elsewhere.
