Yes. On Apollo 15, they left a TV camera behind on the moon that caught the ascent of the lunar module as it lifted off. Here's the video; liftoff occurrs about 2/3 of the way in but it's worth the wait.

Yep, we've got Lunar Module legs parked all over the moon.
They left a camera on some of the later missions to show the take-off from the moon; if you can find one of these clips you'll see exactly what got left behind.

Yes...the base of the lander served as their launch pad. Somewhere out there is an actual video of Apollo 17 blasting off from the moon...taken by a remote control camera which the astronauts set up before they took off. Ah, found a link to it...posted below.
