Olmedo Saenz.
I was at an A's-Angels game a few years ago and the following happened:
2 outs, runnesrs on 1st and 2nd. Saenz was on second. A's batting. I don't remember who hit it, but they hit a bolt into the right-centerfield gap. Darrin Erstad cuts the ball off at the edge of the track and fires it back to the infield. With 2 outs and the force in play, you would assume that Saenz is now reaching home with the runner on first rounding third. The cut off man wheels around to throw to home, and has to double clutch. Saenz is still only halfway to 3rd base. Cut off man fires to 3rd and they get the force. I don't know where Saenz's head was, but not to notice the crack of the bat, not to see the runner on first running up his back, not to hear the 3rd base coach yelling at him to run, not being aware of the game situation.... ridiculous. Needless to say, he did not take the field for the next half inning and was traded at the end of the season.
Complete idiot. Forced out at 3rd with 2 on and 2 out on a line drive into the right-center field alley.

When catcher Kelly Stinnett was with Milwaukee he was terribly overmatched at the plate. I'd never seen a player so helpless at bat.
However, he's still in the majors, with three years in Arizona, three with the Reds, then a year each with the Phillies, Royals, back to Arizona, and now with the Yankees, so he must have something going. He had a couple of reasonably good years in that first stint with Arizona.

Gary Templeton - SS for St. Louis then San Diego. That guy could not find first base with a throw if you gave him a GPS system.
Gary Templeton for Ozzie Smith was one of the top 5 trades the Cardinals ever made a total steal to get the greatest defensive shortstop ever.

Ozzie Guillen - that sorry S.O.B. played the absolute worst defense I have ever seen during the 1999 World Series for the Braves. I was embarrased sitting in my own stadium (Game 2), listening to the Yankee fans berate us. I still get sick to me stomach. See box score - he only had one error charged to him, but he got some lucky scoring as well. Plus he went 0-4 at the plate that night. Now he is an absolute tool in Chicago - go away Ozzie!!!
