Hi Raj,
In male to female transition, the patient will usually have a minimum of 1 year living full time on HRT before surgery. That helps feminise the figure (hips thighs and breasts particularly but also altered fat deposition in the face to a more feminine pattern) but has NO EFFECT ON VOICE, contrary to above notes. It softens skin and thins some body hair growth but electrolysis is needed to eliminate facial hair.
Many TSs go for breat augmentation too as the genetic male chest is broader than the female chest proportionally so breasts are not often as impressive on a broader frame than in female siblings.
Many also opt for facial feminisation surgery (mainly for adams apple, jaw line and nose and forehead contours).
The SRS consists of removal of the testes, degloving of the penis (which the skin is then used to form a vaginal cavity), removal of most of the erectile tissue of the penis and retention of the tip of glans penis to form a sensate clitoris. Some surgeons perfomr this as a two stage procedure followed later by labioplasty as it takes a long time to do, others do the whole job in one operation (although it can take up to 7 hours if that is done).
Yes the girl is orgasmic post-operatively in the majority of cases so yes she can both enjoy being a woman and having sex like a woman but she will have no uterus, therefore no periods and can no longer have children. Also she must take HRT for the rest of her life and certainly for the first few years will need to "dilate" the neovagina to prevent contracture. The average length of the new vagina is 6 inches, equivalent to normal women.
Lubrication can be a problem in some women after SRS, depending on the surgeon and procedure but a little careful planning in the bedroom with lubricants can get around that problem. Many men cannot tell the difference.
There are alternatives, a colon graft can be used and that solves the lubrication problem but is much more dangerous (needless to say it means removing a segment of colong from its normal position and putting it somewhere new).
Also, it is COMPLETELY IRREVERSIBLE. So think carefully and get your decision right if you are going for SRS.
For the female to male, there are various ways to construct a neophallus capable of passing urine but it is not possible to make a spontaneously erectile penis, prosthesis can be used. As for the orgasmic ability, I'm afraid I know little about the female to male procedure to help you there.

It changes a woman or man into opposite sex. If you're a man you are given estrogen hormones voice change to woman's tone and breasts grow then surgeon removes penis. However you will have breasts and voice but NO FEMALE REPRODUCTIVE Organs no Ovaries etc. Females will have DEEPER voice and grown facial hair etc. A penis will have to be made from excess skin. They will loose their breast their reproductive organs will be removed. They WILL NOT GET TESTES. SEX CHANGE SURGERY IS JUST COSMETIC and PIECE OF MIND FOR THOSE WHO FEEL TRAPPED IN OPPOSITE GENDER BODIES.
