You will need to know the states corporation laws to see what all you need to incorporate. You can find them by doing a search on your state laws

It depends upon jurisdiction, but generally, names and addressess of shareholders, dollar value of capital, mailing address of corporation, physical address of corporation, name and address of principal officers for any criminal and civil liability (in corporations liability is passed from shareholders to officers and directors), and the name and address of the incorporator. You may also be required to disclose lines of business.
