The reason they call Edward Elric the "Fullmetal Alchemist" isn't because of the status of his body, but because of his abilities. Ed can transmutate any substance including metal (which is very hard to transmutate. His skills are proven through his little brother Al. As you know Alphonse is mainly a suit of armor and a detached soul. Only a handful worth of alchemist can bond a Human soul to metal armor, and Edward is one of them. And he does it without a transmutation circle. That's why the Furer decided his title should be "Fullmetal Alchemist".

because it is made of pure metal, only metal not a single human part in his right arm and left leg, hardheaded, etc. al is not a state alchemist so he has no codename like his brother but he can use alchemy. the codenames are given to the alchemist by the way they use alchemy(flame alchemist), attitude, special.
everyone has iron in his/her blood, if we don't have we will be weak, to lazy to do work, eat, walk. then die if this prolongs. iron blood for being hardworking, strong endurance, on the front line

it's because of the fact that not only his limbs are metal, but also that he has strong ties with his brother. It's was a nickname made by the producer of the series. Izz in the commentary.

The name Fullmetal is an extrapolation of the original name which roughly means alchemist of steel. It also in Japanese refers to his personality. Stubborn as steel.

The name was given to him when he became a state alchemist.
My guess was that it was due to the fact that the alchemy symbol is attached to the metal hand.

its because metal is his main alchemy power..he seems to make alot of items that consist of metal...like roy mustang..hes called the flame alchemist..and his main alchemy power he likes to use is fire...
