My kid's favorite was a kazoo. You place a square of wax paper on one open end of the roll (tp rolls work best) and secure it with a rubber band. Then all you do is hum into the other end.

If you close both ends securely (Use a circle of cardboard and glue for this.)
and fill with a bit of dry rice or coarse sand you now have a rainstick. I would use the longer tubes from paper towels or even wrapping paper for these.

These can be decorated with anything from paint to glitter or even glue beads on them. It is up to the child's age & imagination.

a kaleidoscope? Cover one end of the roll with clear plastic, put in a handful of sequins or beads, (hey, go nuts and use both!) then cover the inside of the tube again with clear plastic about 3/4 down the tube. so you create like a small bubble and the beads don`t fall out. paint the outside of the tube as colourful as you like and voila!

with instructions?

telescope
periscope
kaleidiscope
light saber
sculpture
telephone

the only instructions a child would need with a paper roll would be found within his/her imagination....just supply some space and some glue....they'll get the rest on their own.

Binoculars. Pencil holders(attatch a cardboard base) or you could use them for wheels on empty boxes to make cars, trains etc. Puppets are another easy one
