I shouldn't answer this question, cause then you would know, and start painting all the bridges and overpasses in your town.
But...... The way we used to do it is by hanging on a rope -- tied in a bowline -- any boyscout can show you how to do that. With two of our buddies on the other end. Best done after 2 am in the morning.

I've always wondered this too.
ladders? Ropes? Cans on sticks? An elaborate mechanical reaching device?
