I think two things people over look are secondary effects of a tragedy. For example, a tsunami, or disease that results from inadequate sanitation. These can both claim many lives - even more than an initial earthquake.

1. I'd guess it's emergency services not being ready and equipped for such a mass natural disaster - thousands of lives are lost that could have been saved just because the services aren't ready for such a disaster.
2. In recent disasters in the aftermass thousands and thousands dyed because they couldn't get clean water and disease flourished in the crowded unclean environment.
