I would very much like to see a completed sample form for 'Application For Informal Probate Of Will And Appointment Of A Personal Representative'. I need this for North Dakota.

It would be inappropriate for anyone but a North Dakota attorney to supply you with that. And, they probably shouldn't, because they don't know the details of your situation.

If you have decided you don't know enough to do it without any help, from looking at samples, you should probably hire an attorney. The reasons are: (A) Attorneys do this all the time. They will charge you, but they don't have a learning curve. (B) If you screw something up, and you are the personal representative, you could end up being held personally liable in many cases; (C) If you screw something up, you may end up paying many times more what you would have originally paid to have an attorney do this for you, to have the attorney fix the problem you caused.

Informal probate is not difficult, but shouldn't be undertaken without a thorough understanding of how your state's probate laws work.

If you need an attorney and don't know one, contact your local or state bar association for a referral.
