So many. the real question is how much do you want to spend and how long do you plan to stay?
I could spend a year or More just trying the best places. the Bellagio has 2 5 star Le Cirque and Picasso, and there is Micheal Mina (formally Aqua) its not 5 star but very close.

I noticed no one mentioned the Top of the World at the Stratosphere. I went there early this year and it's very fine dining with one of the best views anywhere; the restaurant rotates and you can see all of Las Vegas and all the way to the mountains. I've heard the restaurant in the Eiffel Tower is overpriced and not nearly so good. I also love Quark's Bar and Restaurant at the Hilton. It's a totally unique experience if you want a theme restaurant. The actors in alien costumes are amazing. Margaritaville at the Flamingo is a great party restaurant with live music every night and the best beach atmosphere anywhere (but get seated in the main room - outside and upstairs are too far from the action).

I live in Vegas and have been to many different places so to choose a few i would say it depends on what your taste are. I myself don't go to the high priced high scale restaurants like Alex's or spago's, my favorite place to go is basitles hole in the wall restaurant, they have been there for over 30 years and have great Italian food you could die over, it is across the street from Bally's and you need reservation to get a table most nights. Also b.b Memphis barbecue is old style southern food, the ribs are great and the beef is tender. Also Carnage deli inside the Mirage is a good sandwich shop, they serve huge sandwiches and the cheesecake is to die for. Foe steak i would say the golden steer or Ruth's Chris steak house is the place to go.

My personal favorites are: California Pizza Kitchen at Mirage, The Pullman Grill at Main Street Station, Raffles at Mandalay Bay, Pharaohs Cafe at Luxor, Nine Fine Irishmen and Gallagher's at NY NY, Diners Buffet at Treasure Island, the buffet at Orleans, and many others.
It just depends on what you like to eat and how much you want to spend.

All of the big hotels like Mandalay Bay, Caesers, Bellagios, Wynns, Ventitian all have first class restaurant.

i'm going to assume that you are staying on the strip, if not, this probably won't help. here are the best's
outdoors - mon ami gabi at the paris
desserts - lenotre at paris, tintoretto's at the venetian
dinner - wolfgang at mgm, spago at caesars
lunch - nine fine irishman at new york new york
cajun - emeril's at mgm
french - eiffel tower at paris
italian - terrazza at caesars
japanese - okada at wynn
mexican - diego at mgm
steak - smith and wollensky
but the grand prize goes to the delmonico steakhouse at the venetian

Bally's Hotel has the BIGGEST and most delicious buffet. It's fairly expensive, but SOOOOOOOOOOOOOOOooooooooooooo worth it. They have everything from Italian to Chinese to .. well whatever else you want.
