The answer to both, is 'usually'!
You don't need so much space if you put away futons every day instead of having great big beds taking up loads of floor space, or if you don't have large living room furniture ... and Japanese food is designed to be eaten with chopsticks! People DO have - and USE - knives and forks though. I'm sure that chopstick-savvy people are rare wherever you come from, but Japanese people ARE knife & fork-savvy...!

The super rich in Tokyo do not live in small apartments. Most japanese do eat with chopsticks. They are quite useful for eating if you are used to them.

remember everything looks tiny in the TV. it's not actual size. as for chopsticks, generally not at mcdonalds.

not all, there are some luxury units, but you pay for them. Hashi is the prevalent utensil, although some finer restaurants serving European or American cuisine will have forks and knives.
