install a new toilet with an insulated tank or install a mixing valve that will deliver tempered water to refill the tank. the first fix will correct the toilet issue but if the tank is "sweating" the supply piping has condensation on it as well...which will leave dampness on the floor rear l/h side of toilet. you need a plumber to install a tempering valve...it is the best repair. eventually you are going to have problems with floor tile and sub floor.

Either get a small dehumidifier for the bathroom, or insulate the toilet tank. They sell insulated tanks, though I've never purchased a separate insulated tank. I'm sure it's about the same price to purchase the whole toilet insulated. Alternatively, you might purchase some cheap siding styrofoam (thinner the better) and insulate the inside of the tank. This is not an effortless procedure, as you need to completely dry the tank, and use construction adhesive as well as cover all sides of the tank including the bottom with foam without disrpting operation of the ballcock or flush valve and flapper. The easiest way to stop it is to either leave the tank open all the time or have a couple of glasses of hot water handy and fill the tank by hand with hot water every time you flush.

You have to get the temperature of the room compatible to the water in the toilet. Good luck
