Your friend will appreciate a normal birthday more than anything. What did you do last year? Do something along the same lines. Don't draw attention to his bad news or short time left. People are most comfortably with normalcy. Good luck to the both of you.

Find something that your friend has always wanted to do-skydiving, water rafting, going to a certain restaurant. And then try to get all fo that person's friends to pitch in and make it come true. That would really make someone's day.
