Under the handle bars is a "neck" that goes into the frame. Right where the neck goes into the frame there is a threaded nut that can be loosened by turning it COUNTER CLOCKWISE. This will loosen the grip on the neck then you can raise or lower the handle bars/neck. Once you reach the correct height, tighten the nut by turning it clockwise. Good luck.

You can get handlebar risers from most m/c shops which attach to the top of the current triple clamps. I think it would be pointless with clip on bars because sports bikes are designed to have very rigid and direct control of the steering head. I assumed you were referring to motorcycle bars.
Dirt bike bars can usually be rotated inside the triple clamp after loosening the triple clamp bolts. Otherwise just buy new bars with a higher profile and swap over the controls to the new bars.

yes there is a nut at top near goose neck loosen it pull up on hand bars and ajust
