Absolutely. That's why they wore robes with fur collars and cuffs... to draw the vermine away from their bodies. There was even a servant whose job it was to take the lice and bugs off the fur and kill it.

Yes. Everyone was. That is why we have combs and neat hair is a good fashion statement. Fine-toothed combs were used to pull lice off the scalp. Some herbal insect repellents were sort of effective, but they never bathed so lice were everywhere!

I think everyone was. It was pretty gross. I don't think the well to do bathed daily, and lice gets around pretty well from dirty bedding or close proximity to someone else that has it.
