first of all, i want to say that 1984 is very very special book.
i think we dont need the tele-screen to realize that we are being watched. because the culture (all the social systems) is constructed to control human behaviour. ironically, people both created the culture and they are always controlled by it. the man became both a god and a victim of what he created.
whenever you think something is "right" or "wrong", "beautiful" or "ugly", you are watched by an invisible cultural pattern. we cant get rid of this completely even we decide to kill ourselves. after you die, this time, people will make a ceremony for your corpse. so you will be watched carefully while being laid to rest. people will come to watch your corpse at special days.
it is the same while living. everything we wear, everything we listen, watch, eat.. was already ready before we bornt. we just learned what they are, we choose some of them and we rejected some of them. we named it "free will".
now, the culture is still watching us -just like big brother did- and it is still rewarding or punishing us. so who can say that we arent watched ?
i think big brother metaphor has a deeper meaning than using techology. it is not related with industry revolution or results of capitalism anymore. the primary clans dont use a tv, computer, etc. but they are still being watched. because there are rules and values like us, they have some "acceptable" routines or "forbidden" things.
the people have always watched eachother for centuries. that is why all those wars, crimes, ceremonies, rules, values...are seen. it is no matter how you call it for me, big brother, o'brian or else. it is no matter whether you think you are watched or not. you are in this game anyway, whether you believe or not, whether you gain or lose.

I certainly hope so. My life is marvelously boring (I set it up that way so I can enjoy myself), Big Brother would fall asleep after like 20 minutes and the rest of you could get on with your lives without being hassled. You can thank me later. With money. Or gift cards. Whatever works for you.
