What's your question? Do you want me to design a questionnaire for you? Are you suppose to develop a tool for determining if a person has suffered childhood trauma? How to cause a child to suffer trauma? The effects of certain kinds of trauma?

If this is an assignment for a class, my best advice is to drop the class and retake it when you've matured a bit.

I agree, what is your question? Yes a 'questionnaire' is a list of questions, throw the word 'childhood' in and you have a list of questions about childhood, throwing the word 'trauma' in can only be successfully answered if you were the child, because no one else knows everything that may have traumatized you. (not even your parents). Some things may traumatize one person and yet not another. If the memory is not there, then either it was not important enough to be remembered, or it was so traumatic it has been blocked for self protection.
