Self-preservation is part of a human's instinct that demands that the organism survives at all costs. Pain and fear are parts of this mechanism. Pain causes discomfort so that the organism is inclined to stop the pain. Fear causes the organism to seek safety and may cause a release of adrenaline. EGOISM is the view that we are always motivated by self-interest, even in seeming acts of altruism. All human choice is motivated by a desire for pleasure (or an aversion to pain). It can be contrasted both with ethical egoism, which is the view that we always ought to be motivated by self-interest and disregard the interests of the community. It is controversial, since many see it as an over-simplified interpretation of behavior, and it cannot rule out altruism without being contradicted by evidence. Rather, it claims that when sane people choose to help others, it is because of the pleasure they themselves obtain, directly or indirectly, from doing so. Narcissism describes the character trait of self love.

The word is derived from Greek mythology. Narcissus was a handsome Greek youth who rejected the desperate advances of the nymph Echo. As a punishment, he was doomed to fall in love with his own reflection in a pool of water. Unable to consummate his love, Narcissus pined away and changed into the flower that bears his name, the narcissus.

In moderation, narcissism is said to be an essential component of a healthy psyche.

Egoism is mostly of the mind. Someone who thinks too highly of themselves.

Narcissism is mostly concerned with looks. Someone who thinks they are better looking than everyone else.

Well...Adam P. kind of landed the definition with a nuclear approach, and if you remove some of the lingo, he "nailed-it".
It's what you would consider the professional-musician syndrome. (No offense towards the musicians as a whole, just the ones that apply). Your responders have painted a pretty good picture.
Being part of this "inner-circle" of people with a "truckload of talent but can't treat others as a peer," you owe it to yourself to check it out and the definitions will become clear to you.

egoism...is more the excessive concern of one's self- putting one's self first before everyone else ....narcissism has more to do with obsession with oneself- the beauty of one's own body etc...
