I know one really ugly way to do it. You can make an equation with any two points by solving for a and b in y=ax+b, any three points by solving for a, b, c in y=ax^2 + bx + c, any four points by solving for a, b, c, d in y=ax^3 + bx^2 + cx + d. Since you have six points you could create six equations with six unknowns by using y=ax^5 + bx^4 + cx^3 + dx^2 + ex + f by pluging those x, y into that equation each time. Of course then you wouldn't be able to solve it because to my knowledge there isn't a mathimaticaly way to solve a fifth degree polynominal. However I could write a computer program to estimate the values of "a" through "f", but it would take far too long.

If I could spot some pattern, then perhaps it would be much easier to make a formula.

There is an "easy" way of doing this, but I don't know if it will give you what you want:

Using Legrange interpolation:

Let f(x) = 18(x-25)(x-26) (x-28)(x-39)(x-56)/ [(13-25)(13-26) (13-28)(13-39)(13-56)] + 36(x-13)(x-26) (x-28)(x-39)(x-56)/ [(25-13)(25-26) (25-28)(25-39)(25-56)] + 35(x-13)(x-25) (x-28)(x-39)(x-56)/ [(26-13)(26-25) (26-28)(26-39)(26-56)] + 43(x-13)(x-25) (x-26)(x-39)(x-56)/ [(28-13)(28-25) (28-26)(28-39)(28-56)] + 24(x-13)(x-25) (x-26)(x-28)(x-56)/ [(39-13)(39-25)(39-26) (39-28)(39-56)] + 26(x-13)(x-25) (x-26)(x-28)(x-39)/ [(56-13)(56-25) (56-26)(56-28)(56-39)] =

(39391637/ 106159533480) x^5 - (2093630299/ 35386511160) x^4 + (379898703221/ 106159533480) x^3 - (1210337497367/ 11795503720) x^2 + (5679110704087/ 4083058980)x - (5259086912/747813)

you need to give a little more info, that doesn't really explain anything

13,56,26,25,28,39 + 4,70,09.11,14,85 = 18,26,35,36,43,24

but I'm just guessing.
