Lemme see.. by most accounts Dead Alive (a Peter Jackson film, no less) is the goriest movie ever made. Hostel, Saw, Wolf Creek, and all those torture-based films are pretty disgusting as well. The original The Thing is pretty gross. To me, Night of the Living Dead (and its sequels and remakes) is squirm-inducing (try NOT to squirm when the zombies eat human flesh, I mean it's just grody.) Any movie with graphic eye mutilation (Hostel, Zombie, Un Chien Andalou) automatically gets put on the list. Audition is just plain creepy gross torture.

I agree, Hostel, was disturbing, but, with a little less gore, and a little more suspense...I think it would have been outstanding, because, 'those' things do happen...

But, as far as the most disgusting...please watch -- Eraserhead--not only a classic, but, also thoroughly disgusting, and disturbing.

Hostel was very disturbing, that's a scary situation to be in, but i think texas chainsaw massacre has it beat... hanging someone by a hook in their back, chopping off their leg and then putting salt in their open wound takes the cake.

Gotta agree with Bert T . . . Pink Flamingos is truly disgusting . . . of course, it's a John Waters film which means it's funny, too . . . but it's the kind of uneasy laughter you get when you know you're laughing at something that is just so wrong.

"Dead Alive" an early horror film by Peter Jackson [LOTR & King Kong] has some of the most disgusting scenes ever filmed. A true groaner!

Man, I am SHOCKED at all these lame answers. There are literally DOZENS of movies MUCH more disgusting and upsetting than any of the ones mentioned above.

Here are some truly decent movies with some truly unwatchable scenes...

"Pink Flamingos" - Divine, at the end of the movie, in a piece of film just tacked on to the end, is on the street in Balitmore. The camera pans down to show a poodle squatting and taking a dump on the pavement. The camera pans back up to Divine, who drops down on all fours, and, as the dog runs off, picks up a piece of dog crap and eats it on camera. No cuts, no fades, (s)he really did it. Another John Waters movie whose title eludes me right now, but the plot is two neighboring families, each competing for the title "Filthiest Family Alive". The things these two families do to each other, to ensure their own victory, is truly disgusting.
"2000 Maniacs" - A car load of tourists get sidetracked to a small Southern town, inhabited entirely by ghosts of dead Confederate soldiers. They proceed to deal with the Yankee tourists in a number of gruesome ways. One is conning one of the tourists into being sealed into a wooden barrel for a downhill "Barrel Roll Race". After sealing him in, the ghosts hammer a couple dozen nails through the sides of the barrel and give him a push down the hill. This was directed in the 60s by Herschell Gordon Lewis. Another Lewis film features a serial killer who kills and mutilates prostitutes. After killing one black hooker, the killer saws her nipple off with a knife, and CHOCOLATE milk pours out. It's the sickest thing.
"Maniac"- Another 80s slasher flick, effects by Tom Savini. A guy likes scalping models he kills in his apartment. The movie itself is pretty lame, but there's one sequence where the camera just stares at the corpse of a fresh kill, getting scalped. It's so real looking, I challenge you to watch that scene without looking away,

"Faces of Death". There is a whole series of videos, which pick up where your local news leaves off. There are MANY pieces of footage collected by reporters, bystanders with video cameras, etc, which are much too gruesome to show on the nightly news. Suicides, beheadings, train accidents, car accidents, explosions, and, more importantly, the AFTERMATHS of explosions.
Some of the sequences on these videos are included specifically to make you squeamish. Autopsies and the like, and some of the sequences are very obviously faked, but for the most part, a tough series of videos to get through.
Any movie starring Rob Schneider. The man is unwatchable.
