I would assume they do the same thing the do when they delete questions - they just do it. They don't give a warning because they don't have to. I'm sure that, come dawn, you will have nice little e-mail telling you that your questions have been deleted. Perhaps you should give some thought as to how you word things before you post them. If your questions are posted in terms people find offensive they will likely be reported and deleted. Just ask Edna.

Depends on the infraction. If it is questionable you will probably get a warning. But something blatant like "What do you think of this?" then give a link to a kiddie porn site, you will probably be gone (and probably reported to the police).
Not all infractions are equal. Besides, there are some questions I have reported and they are still on here. These are blatant racial slurred or insulting questions too so it probably take a lot to get banned.

Yarr, they did the same thing to me when I asked about a fetus cannibal (serious question). I wanted to know if it was actually illegal to commit the act of...you know. I mean, since the government doesn't consider abortion a type of murder, they're basically saying that a fetus isn't a human. I'm not saying I'm for or against abortion either, I just asked a simple question and it got deleted. (of course, it probably offended most of the people who answered.)
