I filed for protection under ADA due to a chronic disease. Since then, I have been harassed and discriminated against at work. Now, my empolyer wants me to sign a release allowing them to send me to a counsler. I put a notation on the document that the true reason for this release request was not due to medical reason, but as a further form of harassment and discrimination. I've been told if I do not sign another document without anything but my signature and date, I'll lose my job. The form had a section on it marked "comments".

That is a HIPPA violation and carries severe consequences.
Contact a lawyer.

It would seem to me you need legal assistance. Some questions can only be solved by going through the system. They have made their decision. If you allow them to fire you then sue and see if you had the right not to sign. Some chronic diseases could be prevented from working because it would be a danger to the public like anti-social personalities

Did you consider the possibility that you might NEED counseling?
