Go to www.tomcoyote.com and download "hijack this"
Run it and do a scan, find the program with the name of that toolbar, and any accessory programs that have the same name on them and checkmark and delete them.

Be careful WHAT you delete, you can't get it back using this program.

Use 'hijack this' IF it's a hijacker program tool bar, otherwise just uninstall it from the control panel.

What toolbar is it? Many toolbars are in fact spyware and will resist all attempts to remove them. These will require special removal tools and techniques.
