That's not a great example, I've seen some with better pictures... How do they do that??

There are services that help you convert a regular image into this type of ASCII art. Here is one:

You can also copy ones others have done, for example:
http://www.chris.com/ASCII/

Click on the categories on the left side of the page. For example, birthday gives you this:

Just the way you did the above one. When you can do such a design in no time for an example, a person with lot of interest, patience and creativity can make better diagrams.

Basically, the person should have the knowledge of drawing and the creativity to think which charecters to use to bring a particular effect in the diagram. sometimes, the combination of some simple keys produce great diagrams. Its Creativity !
Even you can try, your example was not bad.

Simply by using 'for' or 'while' or similar kinds of loops algorithm by closely observing the pattern of the design.

How it increases and decreases ? Any sequence ? The control factor. Identify these things.

yes, there is no program that does it magicaly, u just type the characters, and it is called "ascii art" so u can google search that and find some mroe.
