Which unhappiness, the one of the people around them, or their own?
In the first case, they don´t see it, because unconsciously human beings do not wish to see it. I mean, if they were to see it, their education would make them think it was their moral duty to help out their fellow mortal and really, they ´lack the time´, ´do not even know that person´, ´do not wish to seem meddlesome´, things like that. They fear that if reaching out without any selfish reason the sky will fall upon their head and they fear that when helping said person, they might discover they´re unhappy themselves.
Why they can´t see their own unhappiness? Some really can´t see it, because they never knew the difference, knew something better, something which made them really happy, others just do not want to think that much about life, saying they wish to live it but instead passing it by, ending up frustrated in the process and lastly, there are the ones who plaster fake smiles on their faces, trying to lie to themselves, trying to lie to others...

qualia, for one. Unhappiness is something that an individual experiences, and that (at least in theory) cannot be shared. To say someone is unhappy when you are not that someone is ... complex. A person who claims to be happy who thinks they're happy, but has a very damaged subconsciousness is one thing, but if people truly are happy at some level, but the rest of the world says they shouldn't be happy (and thus says they ARE unhappy), then it's the rest of the world that is wrong.

Or do you mean more like "Why can't people see the unhappyness in others?" In that sort of case, people's reality revolves around themselves (even the most alturistic person has to use themself as a vessel for alturism). The ability to communicate emotions is powerful, but it's also a "fuzzy science" and people develop emotional communication methods in assorted ways, some of which compliment one another very well, some of which are completely abhorrent to one another.

Because the other people usually hide the unhappiness when they are out with most people. Therefor how the hell are they expected to see it without being psychic

The brain represses their unhappy thoughts. You are unhappy if you believe you are. You are happy if you believe you are. The mind is a powerful thing, When you do not have a cold and you think you do, it could be just the same.

I think that most people are so concerned with their own thoughts and feelings that they are sometimes oblivious to the feelings of others.

Also, a lot of people prefer to focus on the positives, and sometimes they can recognise unhappiness, but don't want to dwell on it. They would rather try to be happy.

If you want people to know how you are feeling, it's your responsibility to tell them. Your life, your mental health is your responsibility, and no one else can be expected to look after that for you. There is help if you need it, but you have to seek it out.
